Paper Patient valuables envelope/form all in one. Recommended for use during a patient admission allowing for a secure storage and record of valuables retain by facility. Size 8-1/2"W X 11-1/2"H with a self sealing closure flap. Each envelope is printed with a sequential number for ease of unique identification. The design of the form allows for listing each valuable separately by line item. The form will produce two (2) tear-away carbon copies. Form information is transferred to front of the envelope providing a permanent record of envelope content. Valuables are placed in envelope and sealed with an adhesive flap. Tear away form may be provided to patient or guardian.
